Step 1:
Review the materials in this tutorial, preferably in the
order provided in the menu.
Step 2:
After completing the last section of the tutorial
(Biosecurity), take a moment of familiarize yourself with
the Withdrawal Time Chart if you have not already done so.
Next, continue to the ‘Training Verification Form’ to
complete the quiz. At the end of the quiz, you can
print the BQA/CMP Training Verification and mail it with the
training and certification fee of $25.00 to SD BQA/CMP, 411
S. Fort St., Pierre, SD 57501. This fee covers the
cost of training as well as certification for 3 years.
Step 3:
After completing the training verification, return to the
BQA/CMP main page and click on
the ‘Site Plan’ button. Next, click on “Word Format”
to generate a printable form. Print the site plan and
complete it with your BQA/CMP certified veterinarian.
After your veterinarian has signed off on the last page of
the site plan, send only the signature page to the address
listed at the bottom of the signature page. Retain the
remainder of the site plan for your records.
After completing these steps, a BQA/CMP card will be sent to
you. A renewal letter will be sent to you every 3
years, at which time your Site Plan should be reviewed and
endorsed by your veterinarian.
